2023 Costume applications are CLOSED for families and WAITLISTED for groups as of 9/27/23. We have reached our maximum requests for the season.

GROUP LEADERS OF REGISTERED 501(C)3 ORGANIZATIONS: Feel free to submit your information using the buttons on the right-hand side below. You will be contacted by a member of The ‘WEEN Team if we can provide costumes for you this year. Thank you for your understanding!

 

Family applications

This application is for mom, dad, grandma, grandpa, aunt, uncle, friend, teacher, foster parent, etc., who are requesting costumes for 1 or more children within the same family. Only one application* is required for families, and shipping costs are covered by ‘WEEN DREAM.

To complete the application, the following information will be required:

  • name

  • age

  • general clothing size (S, M, L, etc.)

  • specific clothing size (2T-3T, 5/6, 12/14, etc.)

  • 2 character requests (one specific and one general)

  • medical information necessary to costume selection, if applicable

  • additional, but not necessary, information: height and weight

group applications

This application is for group leaders, executive or program directors, teachers, principals, etc. who are requesting a large number of costumes for children who are unrelated. There are two stages required for groups and shipping costs are covered by the applicant*.

In order to apply as a group, the following Stage 1 information will be required:

  • organization or school name

  • EIN number

  • contact person’s name, email, and phone number

  • organization’s website

  • number of costumes requested

  • acknowledgment that the organization will cover the cost of shipping*

For Stage 2, the following information will be required:

  • name

  • age

  • general clothing size (S, M, L, etc.)

  • specific clothing size (2T-3T, 5/6, 12/14, etc.)

  • 2 character requests (one specific and one general)

  • medical information necessary to costume selection, if applicable

  • additional, but not necessary, information: height and weight

*The cost of shipping varies depending on the quantity and weight of costumes, the shipping carrier, and the final destination. In 2021, a standard large, Amazon box was, on average $50 per box and contained roughly 20 costumes per box. The organization may choose a shipping carrier based on preference, and may pay using their direct account or by making the monetary contribution through our PayPal account.
