This application is for mom, dad, grandma, grandpa, aunt, uncle, friend, teacher, foster parent, etc., who are requesting costumes for 1 or more children within the same family. Only one application* is required for families, and shipping costs are covered by ‘WEEN DREAM.
If you are local to the New Orleans area, you may pick up your costumes at our office by appointment.

This application is for group leaders, executive or program directors, teachers, principals, etc. who are requesting a large number of costumes for children who are unrelated for 501c3 organizations. For groups, shipping is paid by the requesting organization, school, person etc.
Once your completed application is received, an invoice will be emailed to you for the shipping to be paid through Paypal. Once the payment is received, your costumes will ship.
If you are local to the New Orleans area, you may pick up your costumes at our office by appointment.
